Informed consent
Plentir's preventive health service and the processing of your data.
Before you start, we want to explain clearly what the service consists of, what its limits are and how we process your data. Read it carefully and ask us anything you are unsure about before giving your consent.
This document is a general model drawn up in accordance with Spanish Law 41/2002 of 14 November, the basic law governing patient autonomy. Being general in nature, it cannot cover every circumstance or every possible risk of each individual situation.
1. Who is the controller
The party responsible for the service and for the processing of your data is Plentir Tech, S.L., tax ID B93776417, registered at calle Bárbara de Braganza, 10, 28004 Madrid, Spain.
For any question, to exercise your rights or to withdraw this consent you can write to info@plentir.com.
Your care is provided by Plentir's medical team, made up of licensed and registered healthcare professionals who work in a coordinated way, under shared protocols, within our preventive health programme.
2. What the service consists of
The service is intended exclusively for people aged 18 or over. Its purpose is preventive, advisory and supportive, and it may include:
- Remote consultation with Plentir's medical team by video call, chat, messaging, forms, email or app.
- Preventive health guidance, nutritional advice and healthy-habit recommendations.
- Requesting, coordinating or arranging preventive clinical tests and analyses.
- Interpretation of the results by Plentir's medical team.
- Remote follow-up of objectives, symptoms, parameters and habits.
- Referral to an in-person consultation, a specialist, a further test or emergency services whenever the medical team considers it necessary.
- Use of digital support tools, including automated or artificial-intelligence features, always in an auxiliary capacity and under professional supervision.
For the tests and analyses themselves, Plentir's medical team works in coordination with laboratories and healthcare centres licensed and accredited under the applicable rules.
3. Which tests it may include
Depending on your programme and the medical team's judgement, the service may include tests such as the following. This list is indicative and not exhaustive:
- Blood tests.
- Urine tests.
- Radiological and diagnostic imaging tests.
- Exercise stress tests.
- Cardiological tests, such as the electrocardiogram.
- Metabolic and hormonal studies.
- Microbiota tests or saliva samples.
- Genetic and biological-marker studies.
- Anthropometric and body-composition measurements.
- Sleep and rest studies.
- Health and lifestyle questionnaires.
No test will be carried out without the medical team first explaining to you what it involves. You may refuse any of them at any time.
4. What this service is NOT
The service is not a substitute for an in-person consultation, for specialist healthcare, for a definitive clinical diagnosis, for comprehensive medical treatment or for emergency services.
In an emergency, do not use the platform. If you have severe symptoms, intense pain, difficulty breathing, loss of consciousness, a rapid deterioration or any situation requiring immediate attention, call 112 or go to a healthcare centre.
5. Risks and limitations
You are informed that the service has the following general limitations and risks:
- Remote assessment is limited, as no in-person physical examination can be carried out.
- The quality of the guidance depends on the information you provide being truthful, complete and up to date.
- A referral to an in-person consultation, to a specialist or to emergency services may prove necessary.
- There may be a delay in identifying signs or symptoms that require in-person attention.
- Blood draws or sample collection may cause mild discomfort, pain, bruising, dizziness, a vasovagal reaction or, exceptionally, a local infection.
- A test may need to be repeated because of an insufficient sample or a technical incident.
- False positives, false negatives or doubtful results of uncertain significance may occur, and learning of an abnormal result may cause worry or distress.
- Preventive medicine cannot guarantee the detection or the absolute exclusion of disease.
- Technical faults, connection interruptions or temporary unavailability of the service may occur.
- Digital or artificial-intelligence tools may contain errors or inaccuracies and do not replace professional judgement.
- The use of electronic means entails confidentiality risks, notwithstanding the security measures in place.
- Not following the referral or in-person consultation instructions the medical team recommends puts your health at risk.
The medical team will assess in each case whether the remote service is appropriate. It may not be — for example, in emergencies, with severe or rapidly progressing symptoms, where an in-person examination or an immediate test is required, where a valid sample or adequate communication cannot be obtained, or where any other circumstance arises that, in their judgement, calls for in-person care.
6. Alternatives
You are informed that, instead of this service or alongside it, you can:
- Attend an in-person consultation, see your primary care doctor or the relevant specialist.
- Request a second professional opinion.
- Have the tests carried out at the centre or laboratory of your choice, or have alternative tests where they exist.
- Postpone the tests or not have them at all.
- Not use Plentir's service.
7. Your personal data
We will process your data, including health data and clinical images, in order to manage your appointments, create and maintain your clinical record, provide the healthcare described and follow up on your programme.
Legal basis: performance of the contract (art. 6(1)(b) GDPR) and the provision of healthcare by professionals bound by professional secrecy (art. 9(2)(h) GDPR). For commercial communications and for the advertising use of images, your express consent (art. 6(1)(a) GDPR).
Who accesses your data: only Plentir's medical team and authorised staff, together with the laboratories and healthcare centres involved in your tests and the technology providers serving Plentir under contract and with the safeguards required by the GDPR. Beyond these cases, we do not disclose your data to third parties, except where legally required or where you expressly authorise it.
How long we keep it: clinical data, for the duration of the care relationship and for the period required by healthcare legislation. Data used for commercial communications and advertising images, until you withdraw your consent.
Your rights: you may access your data, rectify it, erase it, restrict or object to its processing and request its portability by writing to info@plentir.com. You may also lodge a complaint with the Spanish Data Protection Agency.
More information: you can find the full detail of how your data is processed in our Privacy Policy.
8. You can change your mind
You may withdraw this consent at any time, in whole or in part, without having to justify your decision and without affecting anything already done. It is enough to tell us in writing at info@plentir.com. Bear in mind that withdrawing consent for healthcare may prevent us from continuing to provide the service to you.
If you do not wish to give your consent, simply do not tick the consent checkbox.
9. Declaration and consent
By ticking the consent checkbox during sign-up:
- You declare that you have read and understood this document, that you have been informed clearly and sufficiently about the nature, purpose, scope, limitations, risks and alternatives of the service, and that you have been able to ask questions and receive answers.
- You declare that you understand that the service is preventive and advisory in nature and does not replace in-person, specialist or emergency care where that is needed; that you undertake to provide truthful, complete and up-to-date information about your health, medical history, symptoms, medication, allergies and habits; and that clinical decisions rest with Plentir's medical team, which takes them according to its professional clinical judgement.
- You declare that you are aged 18 or over and are acting on your own behalf.
- You give your consent to receive the service described and to the processing of your health data for that purpose.
